Periodontal Plan
$55/month or $660/year
This plan is best for patients who have been treated for periodontal disease.
- 4 Periodontal Maintenance Cleanings
- 2 Doctor Exams 3D
- Head and Neck X-ray and Bitewings
- 1 Emergency Exam
- 10% Off Additional Treatment
- 1 Fluoride Treatment
*Payment is due at the time of service
Exclusions & Limitations
- Not transferrable to another party or uncovered family member
- Any service not paid at the time of treatment will be billed at regular price
- Dental exams, cleanings, and x-rays must occur within the year of enrollment and cannot be carried over to the next year
- Discounts under the membership plan do not apply to prior treatments
- Payment is due at the time of service for whitening trays
- CareCredit may not be used to pay annual fees
- May not be used with another in-network dental insurance plan
